As with a lot of Publicity Driven Projects, NASA and Government allowed launch after launch without due diligence to meticulas safety checks, possibly even to the point of an arrogance to brush aside warnings that a problem with "O-Rings" might exist, clearly, tragically, just such a reality therein came to pass. All in the name of Pressure to Deliver. If that isn't negligent arrogance I don't know what is.
