If you break down the show into basics then there is an introverted but narcissistic, New York Oncologist but dirt poor (compared to his wife and father in law), loved by everyone but in truth hated by everyone, Patch Adams but remorseless doctor. Another part is that you are told who the killer is very early but you still have to watch another 5 hours with standard tv show level misdirects before you are told. "Yes! remember the thing you knew at the start? That is the bloody truth". Another part is a lot of artistic nudity. Don't get fooled into thinking this is psychological.
