The best I could make out. Reality and hallucinations are so intertwined that you cannot delineate one from other. Marriage was going bad and husband was having a hallucination of him killing the wife although not purposely. It seems in reality he got killed by wife. The message I think is that a marital relation when it goes wrong beyond a certain extent which it will if there are extra marital affairs can be scary and fatal. Either the husband or the wife can be a victim of domestic violence.
