Excellent and well-laid-out book detailing the history of the “mid-level” (non-physician) provider, prior to the political push to make them “physician replacements” (rather than “physician extenders,” as was originally intended legislatively). In-depth analysis of how almost all research done to date does NOT address how non-physician providers perform on their own (only addresses how they perform WITH PHYSICIAN SUPERVISION).
