Not in the slightest bit realistic until her withdrawals at the very end, when she is in the hospital withdrawing, due to having the opiate blocker injection when she had in fact infected heroin the day before (which she wasn't supposed to do for this reason). The entire film was complete fiction and didn't display any realness at all into the life of a heroin/crack/methadone addict. Very disappointed after the hype.
