The show today related the pharmacist who was a supervion of the person making a deadly error. In my opinion there was not enough Focus upon the medical system such as over working, lack of sleep and the contribution of a system breakdown that overwhelmed the humans creating dysfunction . I am so sorry that we created a scapegoat for a dysfunctional system that created this tradegy. My opinion it is the hospital who is at fault that created this situation and the lack of support to him just further it happened continued to victimize him. Shame on that hospital and what ever legal defense they hired.
