No. A - Doctors treat everyone. They don't turn people away just because they "look" like a gang member or ruffian.
B - Fridging - a sexist trope in TV, film, etc. in which a female character is killed at the start to give a male character the chance to avenge her; also called WiR (Women in Refrigerators). Here, the main guy just needs some motivation to work on his dream, which comes in the form of killing off his wife. I mean, it's not like she worked hard on her dream and made it, became a doctor (which is not easy, as you might know). But of course, go ahead, kill her off like she didn't have a life except for him and his dream.
Hated this. I sincerely request people to stop including sexist tropes like this in the future.
